After nearly 10 years, Donna Graham, nurse navigator with the Madison County Health Coalition is retiring. It is with heavy hearts that we say farewell to this dynamic and dedicated supporter of the Susan G. Komen Ozark Affiliate. Donna was the Komen Ozark 2014 Grantee of the Year. As a breast health navigator, Donna has been an advocate for breast cancer patients and the primary contact for breast health information in Madison County.
Madison County is a Komen Ozark priority area. With only a few area medical providers, historically low breast cancer screening percentages, and more late-stage diagnosis, Donna has worked to fill the gaps. Traveling from Madison County each week to attend breast conference in Fayetteville has been a key component in Donna’s work to identify patients’ needing assistance and tracking their progress. She has served as a consistent coordinator throughout the continuum of care assessing the barriers to healthcare, as well as, the physical, psychological, and social needs of her patients. Donna has monitored women battling breast cancer in her community and triaged their need for services while looking for ways to remove barriers to ensure proper access to services.
